RCI - Usage Fees

If you would like to obtain a copy of the RCI, a usage fee of $50 is required. Once payment is received, the survey will be mailed to you by first class postal mail.

	By PayPal, using the Online Payment Form

Note: Please allow 14 days from receipt of payment to receive your materials by postal mail.
